Here’s how:
STEP 1:Choose the ONE HEALTH CARD package and place your order below.
STEP 2: Upon placement of your order, we will email to you an invoice along with the OHC enrollment form.
STEP 3: Accomplish it, email back to us (scan it or take a photo of it) and send payment.
STEP 4: Once enrolled,, you may pick up your OHC card at the Tyro shop in Quezon City or have it delivered to you (courier charges apply).
